How to file a complaint
If you have questions, concerns, or complaints about a child welfare or juvenile justice issue or case, or if you are worried or concerned about the rights of a child or youth, please contact us by:
- Completing and submitting a Request for Assistance form
- Calling the Foster Care Ombudsman Division at 304-558-1117
- Sending your questions, concerns, or complaints to FosterCareOmbudsman@wv.gov
ALL information provided is kept strictly confidential and only used for investigative purposes.
What to expect after reporting a concern
Your concern, complaint, or issue will be forwarded to staff for intake. You should receive a contact from our staff within two business days. Depending on the nature of your concern, complaint, or issue, you may be contacted for more information or follow-up.
