How to Report Suspected Fraud or Abuse

If you suspect or have information about fraud, waste, abuse, misconduct, or noncompliance involving state officials, state employees, state laws, or contractors providing goods and services to the state related to the Departments of Health, Human Services, Health Facilities, or their programs, you can report it to the Office of Inspector General by:

  • Completing and submitting the appropriate complaint or referral form.
  • Calling the Office of Inspector General at 304-558-2278
  • Calling the Office of Inspector General Fraud Hotline at 1-833-734-0965
  • Sending your complaint or referral via email to OIGIntake@wv.gov
  • Sending a written letter via the U.S. Mail to:
    • Office of Inspector General
    • State Capitol Complex
    • Building 6, Suite 817
    • Charleston, WV 25305

 

You may remain anonymous in making a complaint. Please provide as much detail as possible to ensure we can do a thorough investigation.

 

Please remember that ALL information you provide is kept strictly confidential and only used for investigative purposes. 

 

What to Expect After Reporting Suspected Program Fraud or Program Abuse

All complaints and referrals are forwarded to the appropriate division for review. You should receive confirmation of receipt of your complaint within two business days unless you choose to remain anonymous. Although every complaint and referral is reviewed, not all complaints or referrals result in an investigation or action. 

Depending on the nature of your complaint or referral, you may be contacted for more information or follow-up unless you choose to remain anonymous.

The Office of Inspector General and its divisions does not discuss or release information regarding ongoing investigations.

The dispositions of some investigations may be available on this website on each division’s page or may be obtained by submitting a Freedom of Information Act Request, but not all dispositions are publicly releasable.
